First pest observation worksheet

Turn an unfamiliar bug, stain, or debris pile into a clear record with photos, location, and unanswered questions.

1. Record the setting before changing it

The relationship between the clue and its surroundings can be lost when an item is moved. Start with a wide view.

Write the date, room, and exact surface or object.
Note whether this was a first sighting or a repeat at the same place.
Record recent deliveries, repairs, leaks, or movement of stored items without assuming they caused the finding.

2. Describe only what is visible

Separate a direct observation from a possible explanation. “Three brown capsules under a shelf” is more useful than “a severe roach infestation.”

Note approximate number, shape, color, texture, and whether movement was seen.
Take one context photo and one focused close-up if safe. Keep the original files.
Record a measured size only when it can be obtained without touching or disturbing a hazardous clue. Otherwise write “not measured.”

3. Choose the next question

An incomplete identification is a useful result when it prevents the wrong action.

Use the clue finder to select the closest evidence category.
Compare a plausible alternative, including a non-pest explanation.
Ask what extra observation would separate the possibilities; stop if getting it requires an unsafe inspection.
